ACHIEVE.

General Objective:



To fortify and enhance the health care delivery system by improving the availability, accessibility and quality of health care services as an approach toward improvement and development of health status of the people and community of Barangay San Vicente, creating it into a more accountable, self-reliant, committed and healthy community.


Specific Objectives:


Within four (4) years of community health management, the group must be able to:



  1. Build and create a social connection and bonding with the community of Barangay San Vicente by establishing rapport with the community people.
  2. Perform a comprehensive assessment of the community and community diagnosis through primary data collection by utilizing a formulated survey questionnaire and observational checklist which includes demographic, economic, environmental and social profiles, and by calculating and interpreting health indices and vital statistics through secondary data collection.
  3. Present the survey results and community diagnosis to the residents of the barangay through a community assembly, which includes the analysis of data, and identifying and prioritizing health problems and needs of the community by the people themselves.
  4. Formulate a four-year comprehensive health plan that will aid the people and the community in addressing the identified and prioritized health problem and needs.
  5. Devise, in partnership with the residents of the community, approaches and strategies in solving the identified problems and needs within the context of primary health care system.
  6. Implement the four-year comprehensive health plan with the participation and cooperation of the residents, and in collaboration and coordination with the local barangay officials, municipal and national government, non-government organizations, and various groups.
  7. Monitor and evaluate the progress of the implemented plans regularly.
  8. Document each step in carrying out the schemes for the identified problems and needs for four years.
  9. Adopt and participate in the implementation of any national health programs by the Department of Health.
  10. Apply the different principles of behavioral change communication, leadership, resource management, health information management, and community participation and coordination in implementing the formulated comprehensive health plan.
  11. Document and evaluate the consequence and outcome of the implemented four-year comprehensive health plan.
